Health Screenings 

State-mandated screenings are completed by the school nurse during the school year. They can also be requested by a parent or staff member if they think there may be a problem. Any student that fails a screening will have a referral mailed/sent home for further evaluation by your family health provider or a specialist.  If you do not wish for your child to have these screenings or wish to be present for them, please notify your school nurse in writing at the beginning of the school year. 

  • Height and Weight: Grades K-12 

  • Body Mass Index (BMI): Grades K-12 

  • Vision: Grades K-12 

  • Hearing: Grades K-1-2-3-7-11 

  • Scoliosis: Grades 6 + 7 

Medical and Dental Examinations 

We encourage parents to have these exams completed by their private family health care provider and family dentist.  These providers know your child best and can begin prompt treatment if indicated.  Exams completed on or after July 1st of the year prior to the required grade are acceptable.  Either the physician’s or dentist’s copy of the exam results or the completed and signed forms below will be accepted.  Those students who have not had exams submitted throughout the mandated grade level may have the exam completed by the school physician or school dental hygienist at no charge.  These exams typically occur in April or May and permission forms will be sent home. 

State-Mandated Exam Requirements: 

SMILE PENNSYLVANIA…the Mobile Dentist 

Smile Pennsylvania is an in-school dental program that utilizes the latest technology in portable equipment, bringing dental services directly to our schools. Smile Pennsylvania accepts Medicaid and CHIP as well as private insurance. For those without insurance or the means to pay, they offer a grant program.  If you would like your child to participate in the Smile Pennsylvania Program, please sign up online at www.MySchoolDentist.com or you can request a paper permission form to be sent home with your child if you do not receive one. 

Vaccination Requirements

If your child is exempt from immunizations due to medical, religious, or moral/ethical reasons, a written exemption is required to be on file with the school nurse.  Without having documentation of required vaccinations or an exemption on file, your child may be excluded from attending school. 

Please an updated immunization record to the school nurse when your child receives vaccines.   

Medications in School 

The administration of a prescribed or over-the-counter medications to a student during school hours will be permitted only when failure to take such medication would jeopardize the health of the student and/or the student would not be able to attend school if the medication was not made available during school hours.  Parent consent forms for medication administration must be on file with the school nurse.  ALL medications (even those available over-the-counter) must be accompanied by a licensed health care provider’s order.  Please note, all forms and provider’s orders must be renewed each school year, even if dosing instructions have not changed.  All medications must be picked up prior to the last day of school or the medication will be disposed of, per school policy. 

Serious Health Conditions & Emergencies 

It is important that your child’s information as well as your emergency contact information is accurate and up-to-date with the school nurse.  If your child requires specialized care or a formal health or emergency plan due to a health condition, please be sure to update the school nurse before the start of kindergarten or upon entry into the district and yearly and if any changes occur.
